Community Action Committee of Cape Cod & Islands has certified Health Connector Navigators available by appointment to assist you!

IMPORTANT CHANGES TO CONNECTOR CARE PLANS FOR 2023

ConnectorCare Plans will eliminate co-pays for certain medications associated with Diabetes, Asthma, Coronary artery disease, and Hypertension, as well as primary care visits and mental health outpatient visits.

• Anyone without health Insurance can Apply and Enroll into coverage for 2023. You can Shop and Enroll into coverage for 2023. You can Shop for Affordable Plans, compare deductibles and co-pays.
• Current Massachusetts Health Connector members: this is the time to Review and Renew your plan You can still Shop for Affordable Plans, compare deductibles and co-pays.

Call ahead and Make an Appointment today. Navigators available Monday thru Friday, 8 a.m. – 5 p.m.
